How Much Should I Budget for Health Insurance Before Medicare?
Beyond the Monthly Premium
Health insurance is one of the expenses that makes retiring at 55 or 60 different from retiring at 65. The monthly premium is only part of the number. An early retirement health insurance calculator should also account for deductibles, copayments, coinsurance, prescriptions and the possibility of a year in which medical spending approaches the plan's out-of-pocket maximum.
Personalized Bridge Modeling
This calculator starts with the premium and expected medical spending you enter, then projects those costs from retirement until your selected healthcare transition age. The result is the modeled healthcare bridge rather than a national average.
Individual Household Variation
That distinction matters because two early retirees of the same age can face very different costs. Marketplace premiums vary by location, age and household. COBRA depends on the employer plan. A spouse's plan, retiree coverage or another source can produce a completely different result.
Use Actual Plan Quotes
Instead of assuming that everyone needs $10,000 or $20,000 per year, use actual plan quotes whenever possible.
Should I Compare ACA Premiums or Total Healthcare Cost?
Both Premium & Out-of-Pocket Matter
Both. A Marketplace plan with a lower premium can still have a larger deductible or out-of-pocket maximum. A more expensive plan can have a different provider network, prescription formulary and cost-sharing structure. That is why the calculator separates the insurance premium from expected out-of-pocket spending.
High-Use Plan Cost Context
It also shows a high-use plan-cost scenario using the out-of-pocket maximum you enter. The out-of-pocket maximum does not mean every possible medical expense is capped at that number. Premiums are separate, and plan rules for covered services, networks and other expenses still matter.
Bronze vs Silver & ACA vs COBRA Tradeoffs
This distinction comes up often when early retirees compare Bronze and Silver Marketplace plans or ACA coverage with COBRA: the cheapest monthly premium is not necessarily the same as the lowest total annual healthcare cost.
ACA or COBRA After I Retire?
Federal COBRA Framework
Many people leaving work before 65 have both options available for at least part of the healthcare bridge. COBRA may temporarily continue the employer plan a worker already knows. Under federal COBRA rules, many termination or reduction-in-hours situations can provide up to 18 months of continuation coverage, although plan and qualifying-event rules differ. Participants can be required to pay the full group premium plus an administrative charge.
Marketplace Special Enrollment
Marketplace coverage works differently. Losing job-based coverage can create a Special Enrollment Period, and Marketplace Premium Tax Credits depend partly on household income and family size.
Objective Cost & Network Evaluation
The ACA-versus-COBRA section of this early retirement health insurance calculator therefore compares the premiums and out-of-pocket assumptions you actually enter. It does not declare one coverage type universally cheaper. Provider networks, prescriptions, treatment needs, deductible progress and the timing of a coverage change can matter just as much as the annual premium.
Why Does MAGI Matter So Much After Early Retirement?
Taxable Income Control
Early retirees often have more control over taxable income than workers receiving a regular paycheck. A household may fund spending from cash, taxable investments, retirement accounts, Roth assets or other sources. Those choices can produce different amounts of Modified Adjusted Gross Income, or MAGI.
Net Worth vs Income Rules
Marketplace Premium Tax Credits use household income rules rather than retirement net worth. That means a household with substantial investments can still have a relatively modest MAGI, while a large taxable retirement withdrawal or Roth conversion can move MAGI much higher.
2026 Federal PTC Income Limits
For 2026 coverage, current federal Premium Tax Credit rules generally use an income range from 100% through 400% of the applicable poverty guideline, subject to the other eligibility rules. The temporary enhanced federal Premium Tax Credit rules that applied through 2025 are no longer the 2026 framework. The calculatorAction shows MAGI separately from retirement spending. Spending $100,000 does not automatically mean Marketplace MAGI is $100,000.
Can a Roth Conversion Make My Health Insurance More Expensive?
Tax Bracket vs MAGI Tradeoff
It can affect the calculation. The taxable portion of a Roth conversion generally increases adjusted gross income. For someone using Marketplace coverage before Medicare, that additional income can also increase Marketplace MAGI.
Evaluating Dual Perspectives
This creates a planning tradeoff that appears frequently in early-retirement discussions. A person may have room remaining in a low federal income-tax bracket but also be monitoring an ACA income threshold. Those are different calculations.
Integrated Planning Tools
The Roth Conversion Calculator can model the federal tax-bracket side. This healthcare calculator shows the Marketplace income side so the same conversion can be viewed from both perspectives. Neither result identifies an optimal conversion amount. It simply makes the two effects visible.
Do IRA and 401(k) Withdrawals Count for ACA Income?
Taxable Account Distributions
Taxable retirement-account withdrawals generally can affect Marketplace income. HealthCare.gov specifically notes that IRA and 401(k) withdrawals generally count as income for Marketplace purposes. Investment income can matter as well, including taxable interest, dividends and capital gains where they are included under the applicable tax rules.
Marketplace MAGI Definition
Marketplace MAGI also includes certain items that can be easy to overlook. HealthCare.gov defines Marketplace MAGI as adjusted gross income plus applicable tax-exempt interest, untaxed foreign income and non-taxable Social Security benefits.
Explicit MAGI Entry
That is why the calculator asks for household MAGI rather than trying to infer income from annual retirement spending or portfolio withdrawals. If you are also evaluating early retirement access rules like the Rule of 55 or a 72(t) SEPP series, remember that taxable distributions from those accounts contribute to MAGI.
What Happens If My Income Goes Over the ACA Subsidy Limit?
2026 400% FPL Framework
For 2026, the temporary federal rules that allowed Premium Tax Credits above 400% of the poverty guideline are no longer in effect. Under the current 2026 framework, households generally must have income no higher than 400% of the applicable poverty guideline to receive the federal Premium Tax Credit, along with meeting the other eligibility requirements.
Reconciliation on IRS Form 8962
This makes income estimation particularly important for an early retiree using advance Premium Tax Credit payments. The IRS requires advance credits to be reconciled with the actual Premium Tax Credit on the tax return.
Headroom Visibility & Planning
The ACA MAGI section shows the current modeled distance to the 400% level but does not tell a user to stay below it at all costs. Roth conversions, capital-gain realization, retirement withdrawals and longer-term tax planning can create competing objectives. If you plan to retire at age 55, consider using the Retire at 55 Calculator to model overall cash flows alongside healthcare.
What Changes When I Reach Age 65?
Medicare Planning Endpoint
For most early retirees, age 65 is the major endpoint of the pre-Medicare health-insurance bridge. Medicare eligibility generally begins around age 65, but Medicare is not free healthcare and enrollment rules still matter. Part B, prescription coverage, supplemental coverage and other costs can continue after the Marketplace or COBRA period ends.
Pre-Medicare Bridge Focus
This tool therefore stops the additional pre-Medicare insurance model at the selected transition age rather than pretending medical costs disappear. The purpose of the early retirement health insurance calculator is narrower: estimate the health-insurance and medical-cost gap created by leaving employer coverage before the normal Medicare milestone. When planning later income like Social Security, compare claiming ages with the Social Security Break-Even Calculator.

Early Retirement Health Insurance Questions & Answers
How much does health insurance cost if I retire before 65?
There is no single national amount. Marketplace premiums vary by age, location, household and income, while COBRA and employer retiree coverage depend on the plan. Enter an actual premium quote plus expected out-of-pocket costs to model your own pre-Medicare healthcare bridge.
How much should I budget for healthcare if I retire at 55?
Retiring at 55 can create roughly a decade between retirement and the usual Medicare milestone at 65. The calculator projects the premium and medical-spending assumptions you enter across that period rather than multiplying one national average by ten.
How much should I budget for healthcare if I retire at 60?
The pre-Medicare period is shorter at 60 than at 55, but actual cost still depends on coverage, premiums and medical spending. Enter your age-60 plan estimate and compare the five-year bridge with retiring earlier or later.
Is ACA or COBRA cheaper for early retirement?
It depends on the actual plan costs and Marketplace income. COBRA can preserve employer coverage but may require payment of the full group premium. Marketplace coverage may qualify for income-based Premium Tax Credits. Compare both using your real quotes.
How long can I use COBRA after retiring?
For many federal COBRA qualifying events involving termination or reduced hours, continuation coverage can last up to 18 months. Other events and state continuation rules can differ, so confirm the period provided by your plan.
Can I drop COBRA and switch to ACA whenever I want?
Not necessarily. HealthCare.gov states that voluntarily ending COBRA outside Open Enrollment generally does not create a Special Enrollment Period by itself. Exhausting COBRA can qualify, and annual Open Enrollment provides another opportunity to change coverage.
Do 401(k) withdrawals count as income for ACA subsidies?
HealthCare.gov states that IRA and 401(k) withdrawals generally count as income for Marketplace purposes. The taxable amount and other income should be reflected when estimating household MAGI.
Does a Roth conversion count as ACA income?
The taxable portion of a Roth conversion generally increases adjusted gross income and can therefore increase Marketplace MAGI. This can affect Premium Tax Credit calculations for the conversion year.
Do capital gains count toward ACA MAGI?
Taxable capital gains included in adjusted gross income can affect Marketplace MAGI. The exact tax treatment depends on the transaction and applicable tax rules.
Does Social Security count toward ACA MAGI?
Marketplace MAGI includes adjusted gross income plus certain additional amounts, including non-taxable Social Security benefits, tax-exempt interest and applicable untaxed foreign income.
Is ACA subsidy eligibility based on my net worth?
Marketplace Premium Tax Credit calculations are primarily income-based and also depend on household, coverage and eligibility factors. The value of a retirement portfolio is not itself the Marketplace MAGI calculation.
What happens if my MAGI goes above 400% of the poverty guideline in 2026?
Under the current 2026 federal Premium Tax Credit rules, household income generally must not exceed 400% of the applicable poverty guideline to receive the credit, subject to the other eligibility requirements.
What is the difference between an ACA premium tax credit and a cost-sharing reduction?
A Premium Tax Credit can lower the monthly premium for an eligible Marketplace plan. Cost-sharing reductions can lower deductibles, copayments, coinsurance and out-of-pocket limits, but qualifying users must choose an eligible Silver plan to receive those extra savings.
Should I compare health insurance premiums or the out-of-pocket maximum?
Compare both along with expected medical spending. Premiums are paid regardless of healthcare use, while deductibles and other cost sharing depend on actual services. The out-of-pocket maximum helps model a high-use covered-services year but does not include monthly premiums.
Does the out-of-pocket maximum include health insurance premiums?
No. Monthly insurance premiums are separate from the plan's out-of-pocket maximum. Plan-specific rules also determine which covered services and network costs count toward the maximum.
Can I use an HSA after retiring early?
Existing HSA funds can generally continue to be used for qualified medical expenses. HSA contribution eligibility and the treatment of insurance premiums have separate rules, so the calculator does not assume all early-retirement insurance premiums can be paid tax-free from an HSA.
Are Bronze Marketplace plans HSA eligible in 2026?
Current federal rules expanded HSA treatment beginning in 2026 so Bronze and Catastrophic plans receive HSA-compatible treatment under the new provisions, subject to the other HSA eligibility requirements.
Does health insurance become free when Medicare starts?
No. Medicare can replace the pre-Medicare coverage model, but Medicare itself can involve premiums, deductibles, coinsurance, prescription coverage and supplemental-plan costs. This calculator focuses on the healthcare bridge before Medicare.
This calculator uses user-entered insurance premiums and medical-cost assumptions. It does not quote or sell health insurance. Marketplace Premium Tax Credit estimates use configured federal rules and user-entered benchmark premiums. Final eligibility and credit amounts are determined through the Marketplace and applicable tax rules.
The calculator does not determine Medicaid, Medicare, employer-plan, COBRA, or insurance eligibility. Out-of-pocket maximum scenarios are plan-cost illustrations and may not include premiums, out-of-network services, uncovered care or other expenses outside a plan's limit. Healthcare costs, Marketplace premiums and insurance rules can change annually.
